This is for "CHCPRT001" Assessment 2, Question 3.
"When suspected abuse is identified, services are to fill in relevant forms and notify their local authority to report such suspicions. Download Form 25. Using the form to help you, how would you record the necessary information to demonstrate how you would make this notification?"

What is your answer so far or What have you done so far as an attempt to solve this question?
My Response:
- I would retrieve the relevant information regarding the child himself and the parent from the child’s file, this would be used to fill in the relevant sections of the form requesting information. I would then complete a set of recorded “observations” detailing factually and emotionlessly the information I had seen regarding the child’s hygiene and nutrition, and the information that the child had disclosed to me regarding the eating habits at home. These records would be in the form of written records details in a manner that is consistent with the Child Protection Policy of the service where I am employed.
- I would contact the Department of Child Safety Services to ensure that all of the relevant information required is reported in the relevant and required manner and to receive advice on the matter.

Description and Message:
My response for Question 3 was rejected and I was required to download, fill-out and submit Form 25 as,

"The reason I have asked you to complete the form 25, is because one of the unit of competencies for this cluster, is to demonstrate how you would report indications of possible risk of harm, and you have not provided enough evidence in question 3 to cover all aspects as required to deem this performance criteria as satisfactory."

If someone is able to point me in the right direction to help me get my head around this it would be greatly appreciated. I am happy to supply any other part of my response if that would help .. helping me.
