I need a second opinion of whether i am on the right track or not.

Now i've done an example from the NQS but not sure where i can find any other examples.

So you will need a copy of your centre's confidentiality policy. Have a read through it. On the policy itself it should indicate the regulations and law that it falls under. If not, check out the Education and Care Services National Regulation and Law and you should be able to find some info there. Just Google it...
