An analysis of accidents and near misses over the last 3 months involving both children and staff has revealed a disturbing pattern of possible complacency. The analysis shows:
• Most accidents involving children occurred during outdoor late afternoon play
• Most injuries and near misses involving staff occurred when setting up and packing away equipment (outdoors slightly more than indoors)
• Injuries to staff under 25 occurred at twice the rate as injuries to staff over 25.
• The most common form of injury to children was falls
• The most common form of injury to staff was entrapment of fingers, dropping objects on feet and back strain.

Strategies to achieve desired outcomes:
- more staff supervision outdoors so that there is a less likely chance for accidents to occur
- regular clean-ups of toys left by students to avoid staff injuries while sitting up and packing away . this is also to reduce the number of falls to the children .
- roster of more specific activities for the staff so that things are never left undone and hazards are not made .
Identify existing policies and procedures – what changes need to be made:
- Possibly that all toys and objects to be cleaned up at the end of the day . the changes that need to be made are a constant pickup of toys left alone by students that can cause some sort of hazard.
- There is one supervisor for every 4 children outside . making that less of a responsibility would be to add extra supervision possible 1 to every 3 or 2 as each child has their own interest and needs and needs constant supervision . this will also better for the supervisors for each back-related issues .
