Complacency?
An analysis of accidents and near misses over the last 3 months involving both children and staff has revealed a disturbing pattern of possible complacency. The analysis shows:
• Most accidents involving children occurred during outdoor late afternoon play
• Most injuries and near misses involving staff occurred when setting up and packing away equipment (outdoors slightly more than indoors)
• Injuries to staff under 25 occurred at twice the rate as injuries to staff over 25.
• The most common form of injury to children was falls
• The most common form of injury to staff was entrapment of fingers, dropping objects on feet and back strain.

QUESTION

The Director Trish must decide how to address this issue in order to reduce accidents by 50%. Trish knows that the most effective strategy is to engage the team in developing an action plan.
a) Devise an action plan for Trish to address the issues. You will need to suggest:
b) Desired outcomes
c) Strategies - How to best engage the team in finding solutions
d) How to take into account existing policies and procedures
e) What factors are contributing to the likelihood of accidents/near misses (human/physical)?
f) Address training needs
g) Write a criteria for evaluating the action plan

thats the correct answers for (a,b,c,,d and e)

Desired Outcomes:
- To reduce the amounts of accidents and near misses involving both children and staff.
- Less accident to occur during outdoor play
- Reduce the amount of falls occurring to children
Strategies to achieve desired outcomes:
- more staff supervision outdoors so that there is a less likely chance for accidents to occur
- regular clean-ups of toys left by students to avoid staff injuries while sitting up and packing away . this is also to reduce the number of falls to the children .
- roster of more specific activities for the staff so that things are never left undone and hazards are not made .

Identify existing policies and procedures – what changes need to be made:
- Possibly that all toys and objects to be cleaned up at the end of the day . the changes that need to be made are a constant pickup of toys left alone by students that can cause some sort of hazard.
- There is one supervisor for every 4 children outside . making that less of a responsibility would be to add extra supervision possible 1 to every 3 or 2 as each child has their own interest and needs and needs constant supervision . this will also better for the supervisors for each back-related issues .
What factors are contributing to the likelihood of accidents/near misses (human/physical)?
- unused toys/objects creating hazards
- not enough supervision
- unorganised and unommunicated roles

Hi dear,
I got comment as below:
You are to name the policies example- supervision policy, Hazard item identification, manual handling policy and procedure…
What changes- compare what happens now and what can be change how will you do that.

Identify existing policies and procedures – what changes need to be made:
- Possibly that all toys and objects to be cleaned up at the end of the day . the changes that need to be made are a constant pickup of toys left alone by students that can cause some sort of hazard.
- There is one supervisor for every 4 children outside . making that less of a responsibility would be to add extra supervision possible 1 to every 3 or 2 as each child has their own interest and needs and needs constant supervision . this will also better for the supervisors for each back-related issues .

anyone got any idea how am i suppose to change this?
