What should an educator aim to avoid when making records about children?

It really depends on the nature of the record you are making. Different records may require different entries but on the whole, avoid idol sentence phrases in the record, state facts not assumptions (e.g. it appeared as though J was un well, Ithink she was trying to take the toy away from x) avoid emotion (I feel), avoid ambiguity, exaggerations, personal opinions,information that compromise confidentiality or parents values, delays in making records (record must be recorded as soon as is practicable).
