Documenting Success Of Behaviour and Inclusion Plan

wanted to know how to do this

documenting via a written report of behaviour and iclusion plan of its success and any changes that occured to the plan

So it sounds like you need to write a report on the success of behaviour and inclusion plan and what changes occurred within the plan...

Does that sound right?
