requirements:

we need to compile a portfolio on a child, that show the following evidence:

* Background information on the child's health and well-being using methods:-
i. Running record; or
ii. Anecdotal

* A well child chart relating to the child's age and stage of development.

* a checklist to monitor the child's health and well-being against the well-child chart
