Qualified Assistant Noticing Staff Dont Change Nappy

Hi,
If interviewer ask: u are a qualified assistant in a room/floating in different rooms each day, what u would do if u see some staff don't change nappy?

What is the good answer.

I would probably just look to see if there is a nappy change roster to make sure that you're not jumping to conclusions, maybe mention it to the room leader that you noticed some staff don't change nappies...
