CHCDIV002 - Measurement Metrics

I need help with these last two of the assessment please,

1) explain why outcomes and measurement metrics must be identified and agreed upon.
Who should participate in the agreement and the subsequent evaluations of the strategies and outcomes, and why. What is this likely to achieve?

Someone help with some information and or a website please..
No clue what "measure metric's" mean ither

2) Why is it necessary to evaluate programs and services against desired outcomes?
