Framework For Learning and Development - Diagram 3.16

Under the heading - Behaviour planning for individual children, I need to comment on a chart in K.Kearn's 'Frameworks For Learning and Development'. The question refers to diagram 3.16, on page 126. I can't find this in Edition 3 of that book and I'm hoping someone can provide this for me while I'm on a rare day off and devoting it to study!?

Help will be appreciated :D
