Standards and Protocols When Writing Observations

hi
could you help about this question
what report writing standard and protocol should you follow when writing observation report?
i did not fined direct answer .
I am confused can i write rules of observation RAAVOOM against this question .
Please guide me

Could it be along the lines of privacy and confidentiality policies within the workplace. Writing exactly what you see and hear, not making judgements etc.
