Overview of child and IFSP

Hey there! New here and glad to be apart of this community! I've been trying to become qualified for ages but have had many terrible training organisations, including my current one who wont reply to emails for support

Im currently doing CHCECE021 and trying to develop an IFSP and overview of a child with Cerebral Palsy. Overview of child i'm assuming is name/ age/ allergies/ condition/ support required kind of thing?
IFSP I have no idea as my teacher wont respond but i've read somewhere they're typically for 0-2/3 year olds, Is that correct?
I know parents/ educators/ and support therapists show up to meetings but thats about all i know.

Name of person present at meeting / Role or Agency
All or some of the shareholders i assume/ parent, educator, support etc? I have to write multiple entries so how do i know who should attend and when?
Issues raised by family / Families goals / Families Resources/ Support or Services Requested
How the child will... I assume / Child improving at ... / Unsure on what that means / Inclusion support worker?
Name of Service/ Contact Person/ Phone/ Adress/ Service Provided/ Frequncy/ Services main goal for this child
This also needs multiple entries so im unsure apart from the child care and support workers
