Observing A Child Using A Checklist

I have an assignment where I am to observe a child engaged in both play and routine experiences I have to record the information using a checklist with dated evidence focusing on at least two areas of development.

I was wondering if you are able to help me understand what a checklist with data evidence means. thanks in advance

Sounds like a child developmental checklist based on the developmental milestones e.g - gross motor, social and emotional, language etc. We have a child's developmental milestones which you could use a checklist:

Child Development

Dates evidence will probably refer to how the child did that specific milestone or skill while engaged in play, so you can write what you observed, the date and link it to the checklist...
