Conducting An Inspection Of The Centre

I've been trying to workout what to do for these questions for over a week now but I still have no clue and everyone at the centre just tells me to ask my teacher but I can't contacted her until long after these are supposed to be due.

I was meant to conduct an inspection of the centre with a mentor and complete a workplace risk assessment form and record the results.
First question about this is if anybody knows ...the risk assessment form is that the form from the workplace assessment 3, task 8? Or does the centre give you a risk assessment form?
Second question is how would you record the results?

Another question I was stuck on was "prepared a workplace according to organisational requirement and work objectives". I don't understand how I'm suppose to do this. Nobody even told me what the organisational requirement are or what the work objective is.

Also there's one more question that asks me to review a workplace policy on hazards and to provide a written document with feedback for improvements. Would the workplace policy on hazards be in the policy and procedure book? And the document I'm meant to write, does anyone know what it's supposed to look like and can it be written by hand?

As for recording the results you just add it to the table under each heading...

Could this be referring to the NQS (National Quality Standards)?

For reviewing workplace policy on hazards you can get this from your services policy and procedures manual...
