Safety Issue Report Template

Hi,

I'm required to provide a blank template or sample of a "Safety Issue Report".
I've been trying various word plays to find what I need online yet am coming up blank.
Notification of a potential safety issue, potential hazard, brings up reporting through risk assessment.
I'm certain there is a simplified version out there somewhere that lets an educator say, Hey, the table leg is loose! Can we get the cleaner or owner to bring in a screwdriver?
Anyone able to assist? Due tomorrow!

You can just do a table format in MS Word and add the headings you want!
