In regards of reporting the director and the mother both need to be informed of the incident since the mother specifically mentioned that she didn't want milk to be given to her child.

When discussing this with the mother, should we discuss how milk can supplement a child's diet and nutrition, and the mother's reason for the choice - suspected intolerance etc? Or is this more a case of just respecting the mothers wishes?

You could first find out the mothers reason for the choice and then if it seems reasonable then respect the mothers wishes...
