Any Contributing factors or Hazard

Workbook1, Unit 4, 003 Q1C, Scenario, the accident,

iii. Complete the sample form ‘Employee Near Miss, Accident and Injury Report’ using the information from this scenario.


I'm struggled to find the Contributing factors or Hazard, is it the tiled floor?
