What is a Communication Plan

I have worked in child care now for about 7 years.. mainly as a casual but now full time.
my brother is doing his diploma in child care and one of his questions relates to a Communication Plan which I have never heard of.
I have used communication books for individual children before to share information between home and school.. are they the same sort of thing? if so how and where will I find a communication plan format.

Hi jc1985,
Do you think it could just be a plan of how you plan to communicate?
What you would do on a daily basis.....verbal, communication books, text message, emails. Then what you might do weekly, why and how etc????
How you might communicate if its an emergency and where you would have the numbers etc.
