Ensure Health and Safety

Hello,
I have been trying so hard to find the answer to this question but for some reason I just can't seem to find it and it's starting to really frustrate me

b) Explain what steps you would take if you identified a toy or piece of equipment that presented a hazard with an extreme risk (include reporting procedures)
c) Which policy, procedure or standard does this support?


It would be awesome if someone could help me!

they want to see something similar to these? should i do the same table but only about toys and piece of equipment?

if i understand right i should right about risk matrix and hierarchy of risk control, is it right?
but i could not find out any information about toys and piece of equipment
hierarchy of risk control:
Personal Protective Equipment
Provide suitable PPE to cover and protect an employee
Administration
Implement safe work practices, procedures and policies
Substitution
Replace the activity, process or substance with a less hazardous one
Elimination
Completely eliminate the hazard by removal from the workplace
Engineering
Isolate the hazard from employees with mechanical aids

I'm so so sorry for making mess :) and asking for so much questions....i'm just really frustrated for weeks
Who might be harmed and how?
Soft and hard toys could be vectors for pathogenic organisms.
What are you already doing?
• Hard toys are regularly wiped with antibacterial wipes.
• Soft toys are laundered or replaced.
What further action is necessary? • Ensure Team Leaders regularly undertake cleaning.
