Factors For Recording Disclosure

Hi,

Im struggling with this question and have even rang my tutor to ask for assisstance but she just refered me back to a file in the online library which did nothing to really help! The question is - What are four factors that you would consider when recording disclosure?

My thoughts were
1. Record disclosures made by the child or caregiver.
2. Be objective, stick to the facts
3. Be clear on the basis for your suspicion and include specific indicators
4. Include the date, time, write legibly, sign the entry and print name for clarification.

Does this sound like i am on the right path? Or could someone give me an idea as to what im doing wrong?

Thanks Lorina,

I have read that publication and many others as well. I guess im struggling to understand the question perhaps. There are so many things to consider when recording disclosure. Am i just meant to pick the our that i think are the most important? Is there really a right or wrong answer to this question.... Or maybe i am just overthinking it. I was going to put my above ideas as the headings but then go into a little more detail underneath them all. What do you think?

OK, so the question is asking about the factors you need to consider when "recording the disclosure" (writing what you have been told). Maybe something like:

Immediately after a disclosure you should write down (document *) and date any comments or statements made by the child during the disclosure.
Try to use the child's exact words.
Keep notes about the child's behaviour and emotional state, as well as the circumstances at the time of the disclosure, (e.g. "Child stayed in her desk with her face hidden after all the other children went home. She cried for 15 minutes.")
Be objective and write exactly what you were told and observed. Don't let your personal opinion get in the way...

Basically what you wrote in the first post but I just extended a little... I don't think there are specifically four factors, I think it may be based upon you think are important...

I was actually just having another look at this question myself and came up with the following four factors instead of my first four. I think these new ones are a lot more involved and cover almost all the basic factors collectively. What do you think?

1. The documentation must be factual, objective and accurate and include any verbal or non verbal cues as well as body language and emotion of the child. Ensure you are not using your own opinions.
2. It is important to keep an ongoing record of conversations/disclosures that are up to date including any plans or actions implemented.
3. On all documentation/observations it is important to write legibly, include the date, time, name of persons involved and to sign the document and print your name upon completion.
4. The documentation that I record may be used at a later time by myself or colleagues or in any subsequent response or intervention.
