I'm just wondering if anyone could guide me to answering this question?
question:draft an incident report to management- relating to a recent outbreak of impetigo infections in the service including a review of existing service illness policy.
Undertake some research into impetigo infections and the best practice approach to responding to an outbreak and managing care services with affected children.also undertake a review of your services relevant health policy in order to determine if the existing policy arrangements are suitable or require improvement. Having completed this research and review of existing policy, draft an incident report to the service management providing them specifically you need to communicate:
_ Description of incident and background
_ Particulars of those involved (do not provide names)
_ Immediate action taken
_ Suitability of existing health policies and any recommended improvements
_ Follow up actions planned
My answer
In the last week there have been two reported cases of impetigo infections. the children affected were both
in the same room(2 year old both male) and presented within 3 days of each other (mon and thu). i took the following actions
-Inform the children's parents and exclude the effected children from further care on the day each case presented
and refer for appropriate treatment and management.
- Emphasise the need for good hand washing procedures for all staff and children.
- Advise parents of other children and staff who may have had contact with the cases to remain vigilant for signs of
impetigo and seek treatment if symptoms develop.

could you please help me how should i start i mean how should i address to the Management and what points i should add for
1.Suitability of existing health policies and any recommended improvements
2. Follow up actions planned
(the report should be 1.5 pages)

The following may help:
Impetigo (as a follow up you can add prevention measures).
Health and Safety (page 50, 225 - add exclusion period within the policy for a child who contracted impetigo and child parent should be called immediately if you notice the symptoms.
