Filling out ACECQA Incident, Illness & Trauma Forms

Hi every one

I am just after some advice

On the acecqa forms for illness and incident forms where it has the witness to sign and also the person cometwd the form.

Do they need to be two separate people. I am underthe impression that it needs to be. But my new workplace are doing the same person signing both.


Any help and clarification will be greatly appreciated.

Hi Renae,

I think you are right...

The witness is the person who checks that everything the person filling out the form has written, is accurate. Therefore, it needs to be two separate people - someone needs to check that what has been written is correct, and sign it off.

Yes like Rosie said, I just want to also confirm that when filling out the incident and accident forms that 2 different people will need to sign. One person that fills out the form and another witness. The witness will also need to check the medication/dosage on the illness form and how first aid is being administered on the accident form.
