For the first one - Phone/Email Support this is to be used when you ring or email your assessor/ training advisor. You add the date, brief description of the matter or issue and the outcome.

For the second one - Whenever you study you need to fill it in. Such as the time you studied, the unit you study etc.

These are used for your course provider to keep a track of your self study and to make sure any issues have been dealt with.
