Developing and Reviewing Policies

Develop a questionnaire for stakeholders to check the effectiveness of the original promotion of a policy, their understanding of the purpose of the policy and the clarity and applicability of the procedure section. Ensure you questionnaire are defined so respondents understand the purpose and how their input can assist the review process and will be used..

I really have no idea of what to do here, what questions to ask .... Any help would be very much appreciated.
