Completing A Portfolio For The Nursery Room

Hi, I need to do a port folio in the baby's room but have no idea how to start. I have chosen the child and got consent from the parent. Do I take photos of the child on the first page and obtain a copy of their routine. Do I put daily observation on each page and how do I link this to ELYF?I have questions to answer but not too sure how to present and over what time frame.
