Creating Individual Programs

I need to create 2 individual learning programs (for 2 different children)

I was needing some help with how to set it out and what it needs to include

I'm guessing you will have to take some observations of each child before you can come up with each program...

From the observations you can add extension ideas on the skills, interests and abilities of each child that you had observed through the observationl
