Serious Issue Of Breaching Confidentiality

Can someone please let me know if this is right or help me with some more information, iv looked every where on the Regulations, my text book ect.. The question is, explain why this is a serious issue? if you breach confidentiality in children services?
My Answer is,
By breaching confidentiality you could put the child/children at risk of harm.

Yes, you're on the right track... Also, families have a right to their privacy and if a centre breaches this it breaks down the trust between the centre and the family. It also may cause conflict and issues towards other families if they come to know about an issue (about a specific family) which may have been breached.

can you please let me know one example of where a child’s confidentiality may be breached and what action should be taken in that situation?

my answer is,
taking a photograph without informing parents
is this a right answer

Ok but maybe something like... talking to another Educator about a court order between 2 parents (dad is violent and has a restraining order) a parent overheard and made a complaint to the director that they feel their child is unsafe and what will happen if that parent comes to the centre.
