Scenario: Nail Protruding From The Floor

Hi I am currently on my first book of certificate 3, I am stuck on a question where it has a scenario the children are about to play outdoors and you notice that there is a nail protruding from the wooden floor of the children's cuddy house. then asks 3 questions a) describe the steps you work take upon noticing the nail b) what would you do? C) who would you inform?
but then at the bottom says use the template in your answer. So do I answer all questions and then do the template? Or just fill out the template because it will cover pretty much all of the questions?

I suggest you just fill out the template if the questions are already similar to what it's asking...
