Scenario at workplace

Hi
I have a scenario given which says that Jeremy aged 4 attends childcare and he has a bruise on his cheek .when I ask him what happened he said he hurt it when he fell and bumped his cheek on the step at home later at rest time some bruises on his upper arm are noticed
He is asked again then he says he ran into a tree at the park
Also some raised red mark are across his lower neck which is in the shaped of a belt
Can you please help me write a non judgemental and objective record for suspected abuse based on this scenario

You need to write exactly what you see and what the child says to you without adding your own opinions and ideas...
