observation permission template
Hi i need to type up a permission form to ask permission of parents to include their children in my assignments etc.I was wondering if anyone could suggest a format or there was a template available...thank you

I would just have a letter, obviously from yourself with a bit at the bottom they can fill and sign and then tear off and hand back to you.
In the letter you need to state that it may involve pics of their children...for privacy in assignments you will change names if they want and you can blur out faces in photos ...again if they request it.
I would just write the letter as a letter using your own words as you are more likely to get permissions.
