HLTWHS003 - Final Assessment
1. Access your service’s health and safety policies and procedures. Read the scenario, then use the policies and procedures to help you answer the questions that follow.

Scenario

A parent, Christine Jacobs, is dropping off her daughter at your service. As she leaves, she trips on a worn piece of carpet in the hallway. Christine bruises her knee and the palm of her left hand. You provide assistance and she is soon sitting down with ice on her knee and hand.

Christine says she is fine and there seems to be no reason to call an ambulance. After 30 minutes, Christine decides to leave.

Complete the required reports using:

· the incident/accident report at your service AND

· the service maintenance report or hazard report (use a sample form similar to the following if you are unable to access a service report).
