Joseph is 5 years old and comes to your centre every Tuesday, Wednesday and Thursday. He lives with his mother Lis. You have noticed that Joseph often seems to take the role of caregiver to his mother, who has a series of failed relationships and is currently seeing Jake. Jake has never come into the service. Lis has become particularly close to one of the staff and will often break down and inform the staff member of the fights she has with her partner. Last week, one of the staff overheard Lis recount the events of the previous weekend, when she hadn’t had left the key under the door mat. As a result, her boyfriend broke the front door and came in yelling at Lis. Joseph was still up with her at this time, and Lis continued to say that she was worried Joseph had seen her get hit several times by Jake. Lis appears increasingly concerned that he might do something to Joseph as he keeps threatening her and arriving late at night after drinking. It has been observed that Joseph does not interact with other children as much as he used to and will often lash out if another child approaches his play area.
Just recently, Joseph had come in and sat in the corner of the room, facing the wall for over an hour. Staff tried to approach him however he would lash out towards them then return to the corner. His mother mentioned to staff that he was very upset as Jake had come home late again and there had been some trouble. Later in the morning you were able to talk to Joseph and realised that he had wet himself. In the process of assisting Joseph to get changed, a large purple impact bruise was observed on the right hand side of his back. Joseph said nothing and made no eye contact with you during this time.

1a. Complete the following observation report based on the scenario for Joseph.
The report should contain:
i. time, date, name and signature of staff making the report
ii. full name, date of birth (or approximate age), address and phone number of the child/children you are concerned about.
iii. full name (including any known aliases), approximate age, address and phone number of the parents or carers.
iv. a description of the child ( may be created) and their current whereabouts
v. clear and concise description of the injury/observed harm.
vi. clear and concise observation of the child’s conversation and behaviour (if applicable)
vii. clear and concise records of mothers conversation and behaviour
Personal opinions should not be recorded.
1b. Important for NSW students only You must complete the Mandatory Reporters Guide in relation to Joseph. At the end of the decision report you will be asked to click to generate a report. Do this to open up the PDF report and print or save to your computer to attach with this assessment. You cannot make a report by clicking this it only provides a printed report for your records. All mandatory reporters must call 133627 to make a report in NSW.

the following info may help you answer - PLEASE NOTE, that I am in QLD and in this case there are Regional Coordinators involved who DIrector's report to, whereas some centres may NOT have this type of set up

Principles and Procedures
Principle 1: Responsibilities
Procedures:
1.1 Regional Coordinators are responsible for implementing this policy within their services.
1.2 Service Coordinators/Directors are responsible for ensuring staff and volunteers who have contact with children and young people as part of their day-to-day work have access to relevant information so they are aware of their responsibilities.
1.3 Staff responsibilities are to:
1.3.1 ensure adequate supervision of children and adherence to code of conduct to ensure that harm does not occur whilst children are in care; and
1.3.2 respond to any suspicions they might have that a child has been abused by reporting their concerns to appropriate authorities; and
1.3.3 to inform themselves of:
1.3.3.1 the different types of child abuse and the warning signs that can indicate a child has been abused;
1.3.3.2 the reporting process and the information most helpful to the child protection practitioners working in the Department of Child Safety;
1.3.3.3 the legislative responsibility to maintain confidentiality of specific information that relates to children in care; and
1.3.4 the protection provided by legislation for people who report their concerns about a child to either the Department of Child Safety or the Queensland Police Service.


Principle 2: Expectations:
Procedures:
2.1 Administrators, staff and volunteers have a high level of contact with children in the child care services and play a major part in the successful operation of these services.
2.2 Staff are often seen as role models. They have significant influence on the children they come into contact with and therefore have significant responsibilities.
2.3 Centacare Child Care Services expects that all administrators, staff and volunteers, who participate in a child care service will commit to implementing risk management strategies developed by Centacare Child Care Services for child protection and intervention.
2.4 All staff and volunteers are expected to respond to any suspicion they might have that a child or young person has been abused by reporting their concerns to appropriate authorities (See Appendix 3 – Responding to Disclosures).
2.5 Centacare Child Care Services will support its staff and volunteers to confidently carry out their responsibilities in this regard by providing appropriate guidelines and training materials.
2.6 It is expected that all staff and volunteers will provide adequate supervision of children and adherence to code of conduct to ensure that harm does not occur whilst children are in care.
2.7 It is expected that services will implement age appropriate programs for children around protective behaviours.


Principle 3: Notification Procedures for Allegations of Child Abuse
Procedures:
3.1 Centacare Child Care Services acknowledges that issues regarding child abuse and the reporting of allegations of child abuse is highly sensitive, and should be dealt with in a sensitive, consistent, professional and confidential manner.

3.2 Reporting:
3.2.1 To deal appropriately with these issues and to ensure that the confidentiality of all parties involved is maintained, matters relating to child protection must be referred to the Regional Coordinator.
3.2.2 It should be noted that the employee who reports the matter is not responsible for investigation to a stage where final conclusions can be made. Staff are not to undertake investigations beyond satisfying themselves that they have reasonable grounds to suspect that a child has been harmed or may be harmed. Once a report has been made, the staff member is not required to take further action, beyond the requirement to continue to exercise a proper duty of care.
3.2.3 The Regional Coordinator who receives the report will then act in accordance with this Policy.
3.2.4 It is essential that support and the opportunity to debrief are offered to any staff member who hears or reports disclosures of abuse. The Employee Assistance Program programme should also be offered to any such staff.
3.2.5 The Regional Coordinator is responsible prior approval of any for reporting of allegations of child abuse that occur in the Centacare Child Care Services to the local branch of the Office for Early Childhood Education and Care, and the Juvenile Aid Bureau of Queensland Police Service. A Reporting of Harm Form must be used for this purpose (see Appendix 5). This form must be forwarded to Centacare Child Care Services for signing by the licensee before being sent to the Department. See Appendix 2 for the steps involved in reporting harm or suspected harm.
3.2.6 If the Regional Coordinator is not available then the Consultant – Risk Management and Personnel should be notified.


Principle 4: Investigation of Allegations of Child Abuse
Procedures:
4.1 Investigation:
4.1.1 If an incident of child abuse occurs or is suspected to have occurred in a Centacare Child Care Service it is not the responsibility of persons within that service to take it upon themselves to determine whether the allegation is true or false. It is the responsibility of the Regional Coordinator to notify the Office of Early Childhood Education and Care, Department of Child Safety and Queensland Police Service.
4.1.2 Depending on the nature of the allegations and the extent of the situation, the Office of Early Childhood Education and Care, Department of Child Safety and Queensland Police Service will determine the manner in which the matter will be investigated.
4.1.3 If a staff member interviews a child to satisfy him/herself that they have grounds to reasonably suspect that a student has been harmed there will be two representatives of Centacare Child Care Services present at interviews, where practical.


Principle 5: Confidentiality
Procedures:
5.1 All matters regarding allegations of child abuse are to be treated as confidential. The Regional Coordinator and/or Consultant – Risk Management & Personnel are designated as the primary contacts for any incident involving allegations of child abuse. This is essential for confidentiality. Confidentiality protects all individuals involved:
5.1.1 the alleged victim; and
5.1.2 the alleged offender, and
5.1.3 the person making the notification.

Taj is a 6 year old boy who attends the local After School Age Care Service. Taj is a popular child with both the staff and his peers. Taj is usually well behaved at the centre and likes to participate in most centre activities.
After the last school holidays, Taj returned to the service a changed child, he has become aggressive and picks fights with his peers. He has also become aggressive towards the male staff member and speaks inappropriately to fathers collecting their children from the service. Taj has become a loner. His class teacher has approached the after-school hour service, and was concerned as they also were seeing the same behaviours.
Taj has started wearing jumpers and long pants, even on warmer days and refuses to remove any clothing when he shows clear signs of being overheated.
During play, Taj’s jumper became caught on the table and pulled up revealing a mass of red welts and recent burn scarring on his stomach and back area. It has also been noticed that Taj has started drawing explicit sexual pictures.
1. List the observations, that together indicate possible ‘risk of significant harm’
2. Outline how you would respond to this situation and in preparation to making a report.
3. Describe the actions/work practices you would take as a follow up from the report stage. In your response, discuss record keeping, parent queries regarding the notification and the needs of the child as they continuing attending the service.
4. In relation to Children and Families, what are your ethical responsibilities in this situation? (Access the Code of Ethics to support your answer).

I found Reporting concerns about or young people,guide for professionals handout and our centers policy and procedure guide.

Is this correct.

What have you done so far regarding this?
Think about a child being abused and how they are affected then think about a grown adult being abused? compare the 2 and go from there

Ok, but how does it affect all these people. Does it affect them the same way if they are a child compared to an adult? A child wont understand the way an adult will. Not sure how to put this into writing.
