The Vaccination Letter is a free template for educators in LDC, FDC and OOSH settings to be used for new enrolment applications within the service.
It gives information to new families on providing the child's immunisation history, before the enrolment can be finalised. It also provides information on where to get the Immunisation Status certificate from and who to contact if they are having difficulty obtaining it.
