The Milestones Achieved template can be used as an observation, for child assessment or for half-yearly/yearly reports to detail the milestones that a child has achieved in each developmental domain. Educator comments have also been included. When using this template as a report the Educator can add an overall comment about the child, when using this template as an observation or for assessment purposes the educator comment should include an evaluation or follow up to extend child's developmental learning.
