Risk Assessment Matrix - Fan Not Working

Hello, I have problem to fill in the Risk Assessment Matrix. Need your advise, please...

Hazard: The fan in the infant's cot room is not working

How severely could it hurt someone or how ill could it make someone? (Consequences)

a. Kill or cause permanent disability or ill health

b. Long term serious illness or serious injury

c. Medical attention and several days off work

d. First aid needed

From a to d there are two columm need to fill in which are Assessed Level of Risk and Porbability)

I answered a.Kill or cause permanent disability or ill health
- Assessed Level of Risk is HIGH because of Risk of SIDS and suffocation. Is this correct?

Another Hazard that I found is 'The cord on the electric kettle is badly frayed'
My answer a.Kill or cause permanent disability or ill health
- Assessed Level of Risk is HIGH because of Risk of Electrical Shock and Electrical fire . Is this correct?

You are correct with both answers. They both can cause death, permanent disability or ill health with a HIGH level of risk.

When filling out the risk assessment matrix, I'm pretty sure you just need to pick one option for each particular hazard and state the level of risk...
