The Vaccination Letter is a form to be given to parents of new enrolments. It gives information to new families on providing the child's immunisation history, before the enrolment can be finalised. It also provides information on where to get the Immunisation Status certificate from and who to contact if they are having difficulty obtaining it. This template includes:
