Hi i was just wondering if you could help me out with something.
Today i was going to get parents permission to photograph some of their children for my assignments and i was going to have to ask each individual parent to sign the form i had. I was talking to a lady the other day who did work placement and she said where there was a form that the director could sign to say it was okay to photograph the vhildren as when they were enrolled the parents had to fill out an information sheet saying yes or no to being photographed.
Do you know where i can get the template for a form like this?
