self-assessment information

can someone tell me if I'm doing these good please..
the question is:

what information needs to be recorded from the self-assessment process against the standard and elements of national quality framework?[/color][/color]

my answer is:

* evidence of current good practice
* evidence of what results have been achieved and how your organisation demonstrates that
it meets each standard indicator
* evidence that your organisation meets the mandatory requirements of each indicator, as
relevant
* areas for improvement which will form the basis of the continuous improvement plan
* organisation level rating against each standard indicator.

Organisations are encouraged to make time with their Board or Management Committee to involve them in the process and advise them about steps to address any issues found.

as well i want to add these but I'm not sure..

* the key contacts for the services
*the date of rice of the quality improvement plan
*the date of the assessment and rating visit
*the name of the authorised officer responsible for the assessment and rating of the services.
