Hi,
Apology for posting the same post twice but i realized i had not given it the correct subject line. I'm struggling with this question and have even rang my tutor to ask for assistance but she just referred me back to a file in the online library which did nothing to really help! The question is - What are four factors that you would consider when recording disclosure?
My thoughts were
1. Record disclosures made by the child or caregiver.
2. Be objective, stick to the facts
3. Be clear on the basis for your suspicion and include specific indicators
4. Include the date, time, write legibly, sign the entry and print name for clarification.
Obviously i would expand upon those a bit further but they are the main points. Does this sound like i am on the right path? Or could someone give me an idea as to what im doing wrong?
