Do you have any information about how to create a "Sustainable practices improvement plan" ?

Below are the columns in the chart

Goal- break down into small steps
strategies-actions to achieve the goal
who
when/Time frames
Resources/cost
Reflection

So basically you have to come up with "sustainable practices" that you could do within your service then fill out the table...
