What is systems abuse and what is one strategy that government have introduced to attempt to eliminate or minimise the event of system abuse?

What is your answer so far or What have you done so far as an attempt to solve this question?
The phrase 'systems abuse' is used to describe this. It is defined as
preventable harm [that] is done to children in the context of policies or programs which are designed to provide care or protection. The child's welfare, development or security are undermined by the actions of individuals or by the lack of suitable policies, practices or procedures within systems or institutions.


Description and Message:
Im really not sure if im on the right track with this question. There is nothing in my learning book relating to systems abuse. I have tried google but im really struggling with what systems abuse is and how the government has attempted to eliminate it. Any help would really be appreciated.
