Develop a written plan, for including this child into your children’s service and providing support over a period of time. The way you set out your plan will depend on the child’s particular need. This could range between a high, ongoing need for support or moderate needs.
Your starting point should be the time when the child’s family first expressed interest in your service. Your end-point should be when the child has been enrolled for a sufficient length of time so that development and progress can be documented.

Dad works, Alex* has previously stayed at home full time with mum
1 older sister and newborn (5 weeks old) twin brothers
Recent (2 months) diagnosis of Autism Spectrum Disorder
Mild form of ASD
Type of children’s service: Goodstart Early Learning - Pre-prep room, 5 days a week
Prior to starting:
Have Alex in for a number of transition/observation days
A number of meetings with the parents about what Alex likes/dislikes
Use a developmental checklist to gauge where Alex is at in regards to social, intellectual, physical development.
Have a meeting with the ISP person, centre director and educators in the same room as Alex

Hi, I am struggling to find an example or write a written plan for inclusion of a child with additional needs. I have put what I think is on the right track down, but I just can't find a written plan or anything to help me.
