So, I think you need to follow each step in the Handbook:
Step 1: Preparatory Actions and Detecting Signs of Work-Related Stress
Step 2: Step 2: Identify Hazards, and Groups of People at Risk etc.
Are you currently working at a centre? Or doing work placement?

So, you need to do this:
Once you have completed your assessment, review the results and decide whether it is something you can resolve on your own, or something that would require the support and assistance of your supervisor, your team or the organisation. You should always make it a point to inform your direct supervisor your assessment outcomes and other relevant concerns through an email to ensure that everything is communicated and documented properly. If your concern is something that you do not feel sharing with your supervisor, for whatever reason, you may direct your email to humanresources@whs.com.
Your email must include:
• Completed stress and fatigue questionnaire (required per WHS policy)
• Specific queries or concerns (if applicable)
• Requests or suggestions (if applicable)
• For each of the psychosocial hazards identified during Step 2, review possible solutions. In doing this, look for ideas at the table below. With your supervisor, make a list of possible actions that it would be practicable to implement – either immediately or in future.
• Make a plan that prioritises the various actions agreed on in (2) above; identify who will be responsible for which action and decide on time limits (for example 3-6 months). Where necessary, update your goals as identified in Step 1.
If you are not currently working in a centre, you may just have to make up the results yourself in regards to stress...

Are you referring to this:
Have you looked at the questionnaire in the policy above?
Can u add what you’ve written so I can see where to help you?

Is there a specific stress and fatigue questionnaire, or can I just create one?
