Action Plan For Mentors

Any help would be appreciated....
Develop an action plan for the personal vision and goals for becoming a mentor...
My vision and goals are::
a) build and sustain the personal relationship
b) coaching
c) communicate
d) encourage
e) problem solving
f) conflict management
g) goals, eg) review, encourage building initiatives and encourage advance degree work
I am completely stuck in putting the above in a action plan.
Please help... Thank you
