What do you put in a child developmental profile

Hey guys I am Doing an assignment on bringing a special needs child into the daycare environment, it says I need to make a developmental profile on the child and make an inclusion plan with supporting documentation a from specialists.

So I have a couple questions
First- what is in a developmental profile. I have done a checklist 2-5 years (he's 4 with autism) and I've also done a profile page that has like family structure, allergies, language, comforters etc. what else goes into one of these as I have never dealt with them?

2nd question how do you write an inclusion plan I have asked At my local Childcare centre but they said they can't show me how as it's a breech of policy so is there any tips, templates or advice in how to set it up and what has to be included

Just quickly would you also be able to tell me the module number for the assignment and whether it is a for a cer 3 or diploma so I can add it into the topic heading... Also, please make sure that assignment questions are asked in the correct assignment support forum! :thumbup:
