Mentoring Permission Slip

Hi everyone, I need to develop a permission form to mentor a colleague or student at the Service. I was thinking if any of you could point to the right information that needs to be included in the form, or a general permission form like we have for focus children would be suffice.

So just to clarify... you need a permission slip to gain permission from a student or co-worker so you can mentor them?

If they are over 18 why do you still need a permission slip...sorry haven't heard of this before...

Yes Lorina you have got it right.
But i have never thought this way, if they are over 18, we dont need permission slips. It is mentioned in my assignment that i need permission, may be to document the evidence of the whole mentoring process.

In that case you can have something like:

I (your name), wish to mentor (educators name) for the following period (dates). During my mentoring I will support (educators name) to the best of my ability and share with them my knowledge and experience and guide them within their role.

Then you and the educator can date and sign it...

I'm not 100% sure but that's what I would do if I had to write one...
