Hi,

I am trying to fill out the Reporting of Harm Form 25 for my assessment 2 and am having a bit of trouble. I can't seem to find a sample of a completed form anywhere to help either.
I need to fill it our for the Larry scenario where it is suspected that he is being neglected at home. What I need to know is part 7 of the form is an Outcome report, does this get filled out? It says that the outcome report should outline actions taken at the time of incident and further actions to be implemented to prevent similar incidents from occurring and to ensure the safety ad wellbeing of children at the service in compliance with section 75 of the act. This makes sense if the incident was an accident at the service but for the reporting of neglect, I'm a little confused.
Any help would be hugely appreciated.

What is your answer so far or What have you done so far as an attempt to solve this question?
I assume either this gets skipped on the form or a short letter to state that report has been made and ongoing observations will continue to take place.

Finally found the form! :giggle:

OK so for part 7 of this form it says that the licensee needs to fill in a form/write a letter to be completed and forwarded within 30 days. I think that you should do this part as well since it is a necessity with the overall form. If you can't find the form (did a search can't find it, write a letter). You should include what actions taken and strategies to implement in the future. Even if it is for neglect maybe for future you could add noting down observations earlier, talking to the child on a regular basis about home, talking to family about what is happening... etc.. Sorry I have no idea what the scenario is so these may seem way off!! Anyway you get an idea...

That is a massive help. Thanks so much for that. I have been going around in circles for over a week now with that form with trying to understand it and figuring out the correct way to fill it in. What you said makes complete sense and also makes me feel better that we were both thinking the same things. Thanks again.

Hey Kay2005! I am 'trying' to fill out the same form also. Were you able to locate a mock version of the form filled out? I have had my initial response sent back as I have failed to "demonstrate how you would report indications of possible risk of harm, and you have not provided enough evidence to support this" ... You are clearly aware of the scenario, I am happy to supply the response I have compiled, as I believed it was quite thorough and I cannot find anything else that should be included or considered.

I have encountered significant ... issues ... with the assessment to date due to the correct or incorrect phrasing of the questions and of my responses, so I am hoping I have just worded things incorrectly but the tutor is obviously only allowed to give minimal feedback.

Hi Reisan,

No I was not able to find a mock one online anywhere which I too found frustrating. I had to fill my form out as best I could and hope for the best. In some sections I made up answers such as the names, addresses etc. I also had to fill this out to show neglect of a child so I just took the info that was given in the assignment and wrote it out. I sort of role played the conversation I would have with the child in my head and then wrote it out on the form. There were some parts that I also left blank as they were not really relevant to the neglect scenario. I made sure that all the info that is given was put on the form as I figure you can't really have too much info.
Sorry I can't be more help as I understand how frustrating it can be.

Thanks for that, I too have left some sections of the form blank, and have overdosed the form on information ... *fingers-crossed* eh? :)
