Its an employee injury report form.

Description and Message:
Lets pretend an educator in a child care centre (in Australia) is climbing a ladder, falls, hits her head, lands in an awkward position and is unconscious.

What report forms would need to be completed after this incident and how long does the report forms need to be kept?

I have looked all over the internet for this quite, I am having trouble answering it. Could someone please help me? Thank you.

Kaylaa20, did you end up finding any info for this? I am searching the web looking for the same thing right now, I can't find anything that says how long the form has to be kept for.
