can you point me in the right direction for survey questions for families to identify their needs in their local area.

For this question, I think you need to start by finding out what's available within your local area such as Library (for story time during the day or to borrow books), Aquatic Centre (for swimming classes) etc. and then using this, you can create a list of questions etc.
