The Child's Individual Preferences form is a parent input form and is for parents to fill in and give back to their child's educator. It provides information on the child's food, sleeping and toileting habits to help educators to assist the child as well as devise individual routines and to understand the child's preferences.
This form supports educators and parents to collaborate with one another while caring for the child and to closely establish home routines within the centre environment.
